Location: Crawford County Home Health, Hospice, and Public Health- 105 North Main, Denison, IA
This event takes place MONTHLY on the 1st Monday of the month.
Beginning Monday January 3, 2022 - Saturday April 30, 2022
Crawford County Public Health Immunization Clinics
Appointments are required Please call 712-263-3303 to schedule all appointments
Immunization Clinics are held at:
Crawford County Home Health, Hospice, and Public Health- 105 North Main, Denison, IA
One parent or guardian must accompany the child to the clinic, if someone else will be bringing the child to the clinic; a permission slip from the parent is required. Please bring any previous immunization records to the agency at 105 N Main prior to your appointment. We can only vaccinate children through 18 years of age.
Please wear a mask and only bring child/children that will be getting vaccinated.
