The CMS rated Thomas Rest Haven 2 out of 5 overall, below the Iowa nursing home average of 3.1.
St. Anthony Nursing Home operated as the largest facility in the county during the quarter with 79 beds, followed by Accura Healthcare of Carroll with 65 beds.
By the end of the first quarter of 2026, Thomas Rest Haven had paid $17,215 in fines and received one penalty.
